Forget-Me-Not Commemorative Photograph

You can Honor a friend or loved one with memory loss or dementia or a special caregiver by making a contribution of $10 or more and having his or her name added to our Forget-me-not Commemorative Photograph, which will be featured as part of the 2020 Adams/Sauk County Alzheimer’s Walk fundraiser for September 19, 2020.

Each name will be integrated into the photograph. The photograph will be available locally as a traveling display for at least 8 months following the Walk. All proceeds from the Walk event, including the Forget-Me-Not Photograph go towards local programs and services of the Alzheimer’s & Dementia Alliance of Wisconsin that serves this area and to promote local research towards early detection, treatment and a cure.
