Mission Trip Payment Form
Thank you for supporting the Mission of Hope, Haiti Mission Trip Program. Please be aware that all payments made toward Mission Trips are NOT tax deductible. Fill

Reservation Fee and Payment Information:
• $25 per person reservation fee
• All reservation fee payments are non-refundable and non-transferable
• 50% of total team payment is due 90 days prior to departure date.
• 100% of total team payment is due 30 days prior to departure date.
Note: Team payments should be made as a group, not individually. The team leader is responsible for ensuring the team’s funds are turned in by all due dates.
The below costs apply to all reservation requests submitted after Oct. 1, 2018
- Mission Trips: $650 + $25 reservation fee per person
- Medical Mission Trips: $700 + $25 reservation fee per person
- True North Mission Trips: $995 base price + $25 reservation fee. Additionally, a $100 construction fee per person
